Fertilization failure after IVF: what it means and what clinics may change next
Total or unexpectedly low fertilization can reflect sperm factors, oocyte factors, maturity, laboratory events, or insemination method. A failed cycle can also provide specific information for the next plan.
After conventional insemination or ICSI, the lab checks whether fertilization occurred. When few or none fertilize, the useful question is where in the chain failure occurred.
Conventional IVF versus ICSI
Conventional IVF requires sperm to bind and penetrate the egg. ICSI injects a single sperm into a mature oocyte and bypasses several early steps.

Total versus low fertilization
Zero fertilization and 20% fertilization are both disappointing but can imply different probabilities and next steps. The denominator also matters: one fertilized egg from two mature oocytes is not the same event as one from twelve.
Normal semen analysis does not eliminate sperm-factor questions
A routine semen analysis measures concentration, motility, and morphology, but fertilization is a functional event involving binding, activation, and oocyte response. A normal semen report does not prove that every fertilization step will work normally.

One laboratory value, one embryo label, or one disappointing cycle should not collapse the entire treatment conversation into a single verdict. Fertility treatment is probabilistic. Fertility statistics are easy to make sound better by changing denominators. Fertilization rate is per mature oocyte, blastocyst rate can be reported per fertilized egg or per embryo still in culture, implantation is per transfer, and live birth can be per transfer, per retrieval, or cumulative across a complete egg cohort. When a clinic gives you a percentage, ask for the denominator.
For patients planning more than one retrieval or more than one frozen transfer, cumulative chance across the entire cohort can be more meaningful than the best-looking single-transfer statistic. That is also why avoiding unnecessary embryo discard, unnecessary cycle cancellation, and unsupported add-ons can matter to the overall strategy.
